Whichever plan you choose, you’ll have coverage for your first three network doctor visits - including routine office visits – without having to meet your deductible first.

Co-Insurance	70% / 50%
Office Visit	$35 copay for the first 3 visits (deductible waived). Then 30% coinsurance
Preventive Care	Not covered (see benefit summary for exceptions)
Inpatient Hospital	70% subject to deductible
Outpatient Services	70% coinsurance
Diagnostic Services	70% coinsurance
ER	70% coinsurance
Prescriptions	Greater of $15 copay or 40% Coinsurance. Generic on the formulary list only.
Lifetime Max	$7,000,000
